Managers and applicants must note that all secondments are subject to agreement between the relevant departments and therefore the recruiting manager must speak with HR before offering a job on a secondment basis. * Practice Lead Reablement - CIRT (UCR/Home First & STAR) - Fixed Term until 24th June 2026 Provide management, leadership, and mentoring to unqualified social care professionals and other staff, as directed, who deliver Reablement and Early Intervention as part of a multidisciplinary approach within a social care model. Ensure effective and timely assessment and discharge for individuals within a community setting. Work alongside health colleagues to provide leadership and management on professional social care matters and to promote personalised care pathways, ensuring choice and control for all eligible individuals. Work with others to shape culture and practice, drive continuous improvement, and ensure effective outcomes for individuals and carers. Support the Service Lead by promoting the benefits of integrated working and helping to shape collaboration with health colleagues and other key partners. Contribute as part of the leadership team within the Multi-Disciplinary Team, providing management oversight in conjunction with the Service Manager.
