About the Role
We are looking for Support Workers to provide care and support to individuals living in their own homes across the borough of Wrexham. The role supports the Extra Care Schemes, Homecare and Reablement teams to help people remain in their own apartments or homes by delivering person‑centred care and enabling them to meet their personal outcomes. The care includes assistance with personal daily living activities such as washing, dressing, mobility, physical and emotional wellbeing, maintaining nutrition and meal preparation.
Responsibilities
- Support individuals in daily living activities and personal care to enable them to remain safe and secure in their own homes.
- Assist with physical and emotional wellbeing and nutritional support including meal preparation.
- Deliver assessment services that identify needs for home care and reablement.
- Work as part of a team and on your own initiative to follow care plans and achieve outcomes.
- Maintain accurate and timely documentation of care plans and updates.
Qualifications and Experience
- Dynamic, self‑motivated with empathy and respect for others.
- Ability to work collaboratively within a team and independently.
- Commitment to delivering person‑centred service and following care plans.
- Suitable qualifications for caring roles (e.g., NVQ, City & Guilds, or relevant work experience).
