We are recruiting an experienced palliative care nurse to join the Community Specialist Palliative Care Service.
The post holder will have the opportunity to develop their career within an innovative, collaborative, and experienced multi‑professional team.
The Service is committed to excellent symptom management and high quality emotional support, enabling patients to be cared for according to their preferences.
The team works closely with colleagues across primary, secondary, and social care, as well as the local hospice provider.
The role involves developing advanced skills in the assessment and management of palliative patients, working towards autonomous clinical practice.
This includes contributing to diagnosis, treatment planning, and supportive care, as well as communicating complex and distressing information.
The post holder will work closely with a wide multidisciplinary team to ensure coordinated, high quality care that supports patients and families to achieve the best possible quality of life.

Main duties of the job
The Specialist Nurse provides specialist palliative nursing care for a defined patient and carer population, practicing as an integral member of the multidisciplinary team.
The post holder delivers care across agreed clinical settings and professional boundaries, working collaboratively with statutory, voluntary and hospice providers to support seamless, patient‑centred care.
The role encompasses enhanced clinical assessment and decision‑making, leadership in care coordination, contribution to education and practice development, and engagement with service evaluation and research.
The post holder is accountable for delivering high quality, evidence based palliative care and works towards increased autonomy and advanced clinical competence within their scope of practice, seeking supervision and support as required.

Person Specification
Qualifications
* Registered first level nurse with education to degree level or equivalent.
* Advanced communications skills training or the willingness to work towards this.
* Evidence of continuing education.
* Car driver with current licence and access to a vehicle for work purposes.
* Post‑graduate training or equivalent experience with deteriorating, life‑limiting conditions.
* Advanced communication qualification.
* Completion of uDNACPR (NHS South England).
* Level 6 History Taking and Physical Assessment Qualification.

£39,959 to £48,117 a year. Please note for part‑time hours the salary will be pro‑rata.
