Vine Medical Group is recruiting for a Part time or Fulltime Practice Nurse to join our established Nursing Team, working across three patient-facing sites in Waterlooville.

Our Practice Nurses aresupported by our Lead Nurse and play a vital role in delivering high-quality, patient centred practice nursing services within theboundaries of their scope of practice. The role is varied, our Practice Nurses are responsible forseveral clinical areas such as health promotion, chronic disease management,health prevention, as well as supporting the management team in the reviewingof clinical policy and procedure. They work collaboratively with our teams tomeet the needs of patients and provide excellent and safe patient care.

Were looking for a motivated, organised, and enthusiastic team playerwith a friendly, can do attitude and a strong focus on patient care. If you're keen to develop your skills, were happy to provide training in any clinical areas where you may have limited experience(i.e. travel health).

We can consider candidatesseeking 30 - 37.5 hours per week between Monday- Friday, with a working patternto be agreed to suit both you and the Practice.

To ensure enhanced patient access to the practice we ask our team to support this extended hours working (Saturday mornings or early mid-week on a rotational basis).

Main duties of the job

To provide a diverse and comprehensive range of nursingservices and care to our patient population.

To work in the following clinics/specialities and providehands on, autonomous patient care:

* Cervical Cytology
* Childhood and Adult Immunisations
* Travel Health

A commitment to maintain a high professional standard ofnursing care and to undertake ongoing training and routine skills updates inorder to deliver services.

We operate across four locations; the Forest End, Waterlooville HealthCentre, Aintree Drive and Stakes Lodge sites.We employ a multi-skilled team including GPs, Practice Nurses, RegisteredNursing Associates, Advanced Nurse Practitioners, Healthcare Assistants, ParamedicPractitioners, Pharmacists and skilled administrative teams who support thesmooth running of the Practice.


Job responsibilities

Work within clinical boundaries, demonstrating safeand competent clinical decision-making, delivering high quality clinical careand maintaining patient confidentiality at all times

* Assist and perform tasks related to patient care asdirected by Lead Nurse and GPs following agreed clinical protocols
* To provide essential treatment duties such as BPchecks, NHS Health checks, prescribed injections (e.g. B12 and Denosumab) BMIand lifestyle advice
* Provide holistic assessment of wounds and implementwound care including complex wound management in line with currentevidence-based guidelines such as National Wound Care Strategy(Dopplers/Compression)within own level of competency
* Support NAs/HCAs carrying out wound care bymaintaining clear care plans
* Perform a holistic assessment of patients attendingfor relevant investigatory procedures such as cervical cytology, ECG, followingtraining and in line with local guidelines
* To appropriately request and collect pathological& microbiology specimens including venepuncture, swabs, urine culture inline with local guidelines and process results as required actioning them in atimely manner in line with protocols
* Identify, manage, and support patients at risk ofdeveloping long-term conditions, preventing adverse effects on the patientshealth, promote patients to live healthily and encourage principles ofself-care
* Provide appropriate self-care advice for patientspresenting with minor illness within own level of competency and referring toGP/ANP where appropriate
* Recognise and manage anaphylaxis, according to thecurrent UK Resuscitation Council guidelines
* Perform Cardio-pulmonary Resuscitation and the correctuse of Automated Defibrillator following training, according to the current UKResuscitation Council guidelines
* Provide Travel Health Consultations
* Womens Health including Cervical Screening
* Provide contraception advice and sexual healthguidance. Undertake annual contraceptive reviews and injections in line withpractice policies
* Offer advice about childhood and adult immunisationsand ensure vaccines/injections are administered under patient group directives(PGDs) or individual prescriptions (PSDs)
* Provide chronic disease clinics, delivering patientcare as necessary, referring patients to secondary / specialist care asrequired
* To provide general health promotion education, adviceon national screening programmes and guide/refer patients to appropriateservices or other health professionals as deemed appropriate
* Provide information and advice on prescribed orover-the-counter medication, on medication regimens, side effects andinteractions as appropriate and within your level of competency
* Prioritise health problems and intervene appropriatelyto assist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care
* Support the clinical team with all safeguardingmatters, in accordance with local and national policies
* Accurate recording of care provision onto patientrecords ensuring correct coding of all procedures and information usingapproved methods and templates
* Chaperoning and assisting patients where appropriatewho are being examined by another clinician
* Vaccine / cold chain storage, monitoring and recording
* Conduct routine and post-use inspections of emergencyequipment and medications
* Ordering of clinical supplies and equipment, assistwith stock checking and maintaining the equipment inventory
* Clearing and re-stocking clinical rooms/trolleys atend of the session ensuring stock rotation and date checking
* Cleaning and maintaining clinical equipment, reportingfaults promptly
* Preparing and maintaining environments and equipmentbefore, during and after patient care interventions
* Actively participate in all aspects of infectionprevention and control and champion this within the practice
* Helping to raise awareness of health and well-beingand how it can be promoted
* Assisting with the collection and collation of data onneeds related to health and well-being and support QOF requirements and datacompletion
* Actively participate in seasonal and special projectsas requested e.g. flu campaign
* Maintain accurate clinical records in conjunction withPractice policies and current legislation


Person Specification


Experience

* Experience as a Practice Nurse in Primary Care, following agreed clinical protocols with referral to GPs as appropriate
* Experience of chronic disease management
* Post graduate experience in one or more areas of chronic disease


Qualifications

* NMC Registered Nurse
* Post graduate diploma or degree (in one or more areas of chronic disease)


Knowledge & Skills

* Clinical skills and knowledge: Wound care, Urinalysis, ECGs, Venepuncture, Hypertension, Cardiovascular disease primary prevention, Coronary Heart Disease, Diabetes, Routine immunisations/childhood immunisations, Requesting pathology tests, for example urine culture, swabs
* Excellent organisational and time management skills
* Able to work as a team member and autonomously
* Excellent communication skills, able to effectively engage with a diverse range of people facilitating differing communication styles and needs
* Full UK Driving licence with own transport
* Confident IT user with accurate data entry skills and a working knowledge of Windows and Office 365
* An understanding acceptance and adherence to the need for strict confidentiality
* Working knowledge of EMIS
* Understanding of QOF
* Clinical skills and knowledge: Able to remove coils, Ring pessaries, LARC fitter, Travel medicine, Spirometry, Cytology
