Description Assistant Practitioner | 10207 | permanent contract | 37 hours per week | £32,061 to £33,699 per annum | Grade G | Great Yarmouth An exciting opportunity has arisen for us to recruit an Assistant Practitioner to join the Eastern Community Older People and Physical Disability Team, covering central Great Yarmouth and the northern villages. Early intervention, creativity and utilising community assets are key, as is improving our community led support offer that will include engaging with people in public spaces. You will undertake Care Act Assessments, Support Planning, Reviews and take part in covering our duty system whilst working collaboratively with partners and stakeholders to promote independence. We are seeking an enthusiastic community Assistant Practitioner to join our highly motivated, innovative team of Social Workers, Occupational Therapists and Assistant Practitioners. Our team is experienced with working with complex cases, have a sound knowledge of the Care Act (2014), Mental Capacity Act (2005) and Safeguarding, as well as excellent time management and IT skills. Any new staff are supported to develop a thorough working knowledge of Norfolk County Council processes and procedures. Our team of Social Workers, Assistant Practitioners and Occupational Therapists use the Care Act (2014) to assess and provide support to adults living in the community. We are investing in people who can take us forward on our journey to embed our vision. As an Assistant Practitioner within this team, you will undertake assessments, service commissioning and reassessment work. These will be carried out via video/telephone and/or face to face. You will have excellent communication, record-keeping and keyboard skills, along with experience of working with vulnerable people. In addition, you should have a commitment to excellent customer service and be familiar with the Departments' eligibility criteria.
