Overview
An exciting opportunity has arisen for two Clinical Pharmacists to join the North Solihull Primary Care Network (full time and part time considered). This PCN covers a group of GP practices serving a population of around 80,000 across Chelmsley Wood, Smithswood, Kingshurst, Sheldon, Marston Green and Castle Bromwich. The PCN aims to enable practices to work together to deliver network-based services and improve patient care.
Responsibilities
* Work as a clinical pharmacist within the GP practices in the PCN, supported by a GP lead/mentor, and travel between surgeries to ensure equitable access to the service.
* Work as part of a multidisciplinary team (GPs, nurses, HCAs, physiotherapists, paramedics, social prescribing link workers, Health and Wellbeing Coaches, administrative staff) to provide holistic care and medicines expertise.
* Take responsibility for chronic disease management and undertake clinical medication reviews to manage patients with complex polypharmacy. Focus areas may include CVD, COPD, Asthma, Diabetes, Frailty, and related conditions.
* Provide primary support to general practice staff on prescription and medication queries, conduct reviews in clinics or remotely, support the repeat prescription system, handle acute prescription requests, and perform medicines reconciliation on transfer of care for safer prescribing.
* Run minor ailment clinics in surgery where appropriate and offer medicines advice to address public and social care needs of patients in GP practices.
* Provide clinical leadership on medicines optimisation and contribute to quality improvement, including aspects of the Quality and Outcomes Framework and enhanced services.
* Work to integrate with community and hospital pharmacy to optimise skill mix, improve patient outcomes, and help manage workload within practices.
* Be driven to develop the role, with potential progression to non-medical prescribing where appropriate.

Requirements/Qualifications
* Registered Clinical Pharmacist eligible to work in primary care settings (within professional boundaries).
* Ability to work autonomously and as part of a multidisciplinary team; willingness to travel between surgeries.
* Interest in chronic disease management, medicines optimisation, and running pharmacist-led clinics.
* Desire to develop clinically, including potential non-medical prescribing where appropriate.
