We are seeking a confident and experienced Qualified Social Worker to join Derbyshire's Short-Term Assessment & Reablement Team. This is a fast-paced, front-line position suited to someone who is skilled in handling complex cases and able to work both independently and collaboratively within a supportive team.
Location: Floor County Hall, Derbyshire Dales South P&P Team
Pay Rate: £33-£34 per hour (depending on experience, umbrella)
Key Responsibilities:
Undertake Care Act assessments and develop person-centred care plans
Manage safeguarding enquiries and contribute to MARM processes
Complete Mental Capacity Assessments (MCA) and Best Interest (BI) decisions
Work directly with service users and carers to promote independence and positive outcomes
Participate in Duty cover 1-2 days per week
Conduct face-to-face visits as required
Requirements:
Qualified Social Worker (Degree/DipSW/CQSW)
Current Social Work England registration
Previous experience within assessment or reablement teams
Ability to work at pace and manage a varied, complex caseload
Car driver with access to own vehicle (essential due to community visits)
