KEY DUTIES AND RESPONSIBILITIES
* Undertake daily living tasks as agreed with the service user, their family, and relevant professionals.
* Work within the policies and procedures of Connected Living.
* Follow care and risk assessments tailored to each service user's needs.
* Perform Health and Safety checks in line with policies.
* Work confidently alone in accordance with lone worker risk assessments.
* Ensure safe medication delivery and management at service users' homes.
* Respect service users' preferences in task execution.
* Promote choice and independence for service users.
* Manage behaviours in the least restrictive way possible.
* Complete all documentation, notes, and handovers efficiently and accurately.
* Support clients' emotional and physical wellbeing as part of a team and wider multidisciplinary team.
* Develop and maintain professional relationships with service users and team members to ensure positive outcomes.
