Rehabilitation Manager – Managed Long-Term Care (MLTC)
Location: Kew Gardens, NY.
Hybrid schedule: 3 days in-office, 2 days remote, with home visits as needed. Coverage area includes Queens, Long Island, and North Brooklyn.
Job Type: Full-Time/Permanent.
Overview
We are seeking a Rehabilitation Manager to support the clinical oversight and delivery of rehabilitation services within Managed Long-Term Care (MLTC), MAP, and ESNP programs. This role focuses on ensuring members receive appropriate care and equipment to support independence and quality of life at home.
Responsibilities
* Review rehabilitation documentation from external providers to ensure services are clinically appropriate.
* Evaluate and process requests for Durable Medical Equipment (DME) for home use.
* Provide education and guidance to nursing leadership regarding rehabilitation services and care planning.
* Perform home and field assessments as needed (including evaluation for complex DME and home safety).
* Maintain compliance with New York State Department of Health regulations.
Qualifications
* Master’s degree in a healthcare-related field.
* Active New York State Occupational Therapist License (required).
* 2–4 years of experience as an Occupational Therapist.
* Strong communication, organizational, and computer skills.
* Ability to work effectively in a fast-paced, collaborative environment.
* Willing to conduct home visits.
* Must be able to travel within Bronx, Manhattan, and Westchester regions.
