We are recruiting a Dietetic Advanced Clinical Practitioner to support people with diabetes and the existing team. This role will be based at the Leicester General Hospital and will cover Leicester, Leicestershire, and Rutland.
Come and join us in this new and exciting venture, blending medical nursing and dietetic services together to support the diabetes virtual ward, bridging the gap between hospital and primary care. We believe in delivering safe, effective, and personalised healthcare at the patient's own home, working with diabetes specialist nurses supporting people with diabetes in their homes. The role will be predominantly based at Leicester General Hospital with possible cross-site working.
The Diabetes virtual ward aims to help people stay well and at home through self and supported monitoring. Running successfully for two years, the service is expanding to include direct referrals from East Midlands Ambulance Service and potentially a Diabetes SDEC service. Funding is secured until March 2026, with positive outcome results indicating continuation beyond this period.
You will provide comprehensive dietetic assessment, treatment, and management as part of the multidisciplinary team in diabetes. The caseload is varied, covering all aspects of diet and nutrition, with consultations conducted both in person and remotely, including some home visits. You will work closely with the Advanced Nurse Practitioner Lead to develop and evaluate the service.
The role involves leading as the Clinical Specialist Dietitian responsible for the Nutrition and Dietetic Service delivery to the adult Diabetes Virtual Ward, managing a complex caseload independently, and exercising a high degree of autonomy.
Key responsibilities include conducting complex nutritional assessments, developing dietetic care plans based on biochemical, anthropometric, and clinical data, diagnosing malnutrition or metabolic crises, making clinical decisions on nutritional support, and leading on service development, policies, and training.
You will also be an expert in using anthropometric equipment and enteral feeding pumps, training others, and implementing new nutrition products with evidence-based support. Providing expert advice to patients and healthcare professionals, practicing according to professional standards, and contributing to multi-professional meetings are essential aspects of the role.
