Location: University Hospital Wishaw. The Frailty Unit provides specialist multidisciplinary care for older people living with frailty, focusing on early assessment, rehabilitation, and safe discharge planning whilst applying home first principles. Responsibilities
* You will contribute to the care of patients admitted to the Frailty Unit, including provision of pharmaceutical care, medication advice and prescribing.
* These exciting new roles will contribute to the seven-day Frailty Service to ensure patients receive high quality pharmaceutical care.
* You will become an integrated member of the consultant-led multidisciplinary team, liaising with primary care, hospital and community colleagues.
* There is a requirement to be an established practicing independent prescriber due to the complexities of patients presenting within frailty; prescribing and de-prescribing will be an integral part of this role.
* Full and part time hours will be considered.
Qualifications and requirements
* You must possess a Masters Degree in Pharmacy and be registered with the General Pharmaceutical Council.
* You must have an independent prescribing qualification or be working towards this.
* Evidence of postgraduate training and professional development in clinical pharmacy practice.
* Relevant post registration experience in clinical pharmacy.
* Be able to participate in pharmacy emergency duty service.
* Ability to work flexibly to cover a rotating schedule over 7 days, including weekends and public holidays.
