Job summary

We are seeking a committed and motivated Specialty Doctor in Psychiatry to join our dynamic mental health team at HMP Bronzefield, the largest women's prison in Europe. This is an opportunity to deliver meaningful care in a unique setting – supporting some of the most vulnerable individuals in society.

As the Specialty Doctor you will play a vital role in the delivery of psychiatric care within the prison. Working closely with a multidisciplinary team, you will assess, diagnose and manage a variety of mental health conditions. The role offers rich clinical variety and professional challenge in a secure setting where your input will be valued and impactful.


Main duties of the job

The vision for Health & Justice is: "Working in partnership with the criminal justice system and other agencies, we will provide responsive, dependable and high‑quality primary care, mental health and substance misuse services to existing offenders (and those at risk of offending) and non‑offenders in the communities we serve, to improve their health status and to encourage social inclusion."

The specialty doctor will provide clinical leadership to ensure the delivery of high quality patient care to the men/women resident in the Surrey prisons (HMPs High Down, Downview when it re‑opens, Send and Coldingley). You will develop your own specialist interests and, where appropriate, bring these in line with Trust and local service development plans. Teaching and training of medical students, specialist nurses, other nurses and prison staff will also be part of the role.


About us

The Health & Justice Directorate is one of five Service Lines within the Trust providing clinical services. The service has grown quickly in recent years and currently comprises a range of innovative prison, police and court services, a small number of low secure inpatient beds and specialist community forensic teams. Our services stretch across six London boroughs, plus Kent, Surrey and Buckinghamshire.

CNWL's Health & Justice Service Directorate is experienced in applying for prison and secure setting contracts. The post holder will be encouraged to participate in the ongoing development of current services and bidding for new prison health contracts, with business, finance and administration support centrally.

We currently provide a range of services (including mental health, substance misuse, sexual health and primary care) to the following prisons: HMPs Bronzefield, High Down, Downview–Coldingley, Feltham and Woodhill. We also provide community forensic services in North West London, liaison and diversion services to police stations and magistrates' courts (Uxbridge, Hillington, Willesden and Westminster), to the Central Criminal and Harrow Crown Court, as well as low secure specialist forensic services at Park Royal. The service line is led by Dr Mellisha Padayatchi as the Clinical Director, and by Patrick Gillespie as Service Director. Operational Accountability for the post would be to the Service Director.


Job responsibilities

Clinical: Assessment (including risk assessment), treatment, care planning and multi‑agency liaison for people with mental health problems in the prison. CPA management of care. Undertake mental capacity assessments. Contribute to planned emergency interventions. Liaison with prisons, courts, statutory and non‑statutory agencies concerning all aspects of mental health care management of men with mental health problems in the prison. Assessment and recommendation under the 2007 (Revised) Mental Health Act (the post holder must maintain Section 12 approval). Work closely with the Manager, Psychologists and other lead professionals to provide clinical leadership and ensure safe, supportive care. Liaise with other medical, clinical and managerial staff to provide high‑quality care pathways. Participate, when relevant, in ACCT (self‑harm / suicide prevention) processes. Practice in accordance with NICE and other relevant guidelines and CNWL Trust policy. Accurate record keeping in line with Trust and professional guidelines (data protection legislation and information governance standards). Clinics will be allocated to prisoners on ordinary location, healthcare and those housed within the segregation units; demand for assessment on healthcare and segregation is highly variable, so some flexibility is required.

General: Contribution to care quality through audit, research and participation in local and Trust‑wide care quality processes, including CQC inspections. Practice in accordance with NICE and other relevant guidelines. Contribute to ensuring that the Addictions and Offender Care Directorate and the Trust as a whole achieve the activity, performance and outcome targets as agreed with key stakeholders. Contribution to service development (care pathways, clinical protocols, new service development) according to need and in agreement with the Consultant Psychiatrist.

The post‑holder will recognise that the description of clinical duties is not exhaustive. The detail of clinical duties will be reviewed in light of the post‑holder's experience and the needs of the service. Amendments may be possible as the Directorate continues to develop.


Person Specification


Education and Qualifications

* Primary Medical Degree
* Full GMC Registration
* GMC Licence to Practise
* MRCPsych (or equivalent) – desirable
* An additional postgraduate qualification of relevance to own specialty – desirable
* Section 12 MHA Approved – desirable


Experience, Skills and Abilities

* Completed at least 4 years full‑time postgraduate training (or equivalent part‑time or flexible basis) with at least two years in a specialty training programme in a relevant specialty or as a fixed‑term trainee or with equivalent experience and competencies
* Published audit or research – desirable
* Interest in teaching and training medical and non‑medical colleagues – desirable
* Experience in service development – desirable
* Evidence of evidence‑based medicine training, teaching and implementation – desirable


Personal Skills & Qualities

* Excellent communication and empathic skills
* Demonstrable leadership skills and experience of team working
* Sound organisational skills
* Ability to work on own initiative
