Job overview

To oversee and provide assessment, planning, implementation and evaluation to service users within a variety of settings, and ensure records associated with this process is maintained. To work effectively within a multi-disciplinary team and across other agencies and to provide a high quality of care to individuals with learning disabilities and complex mental health and social needs. The post holder will carry out nursing procedures to a standard that ensures safe and effective care, and complies with

The Nursing and Midwifery Council's Code of Professional Conduct. The post holder is not responsible for clinically supervising junior staff but will be expected to be involved in supporting others as well as students. There will be an expectation that the post holder

will facilitate user involvement in all aspects of service delivery.

Main duties of the job

To provide specialist nursing input for service users with intellectual/learning disabilities and mental health needs. This will include assessment of needs and risk, implementation and evaluation of care in conjunction with the service user, the carer, MDT and other agencies

To act as Care Co-ordinator under CPA for a defined group of service users

ensuring effective liaison and consultation with them, their families and carers, GPs and others services about support, management, possible risks and contingency strategies

To liaise with, advise, and support carers, community based mental health

workers, inpatient services, support workers and primary healthcare team

members on the needs of service users with a learning disability and mental illness

To ensure that all service users have documented care plans which are reviewed and evaluated on a regular basis

To attend clinical meetings, CPN meetings, high risk complex case meetings, and other meetings as appropriate.

To mentor allocated RMN/RNLD student nurses.

To work effectively within a multi-disciplinary context whilst maintaining the unique perspective and contribution of nursing.

To undertake regular supervision of own case load with team leader

To act as an accountable professional in line with the NMC code of Professional Conduct and with relevant legislation.

The Behavioural and Developmental Psychiatry CAG provides mental health services for adults with neurodevelopmental disorders and those who require enhanced risk management. The CAG is led through integrated leadership with the joint Clinical Directors and Service Director. Enabling leadership from our front line teams is at the heart of our CAG vision. Our lead management structure supports this with the addition of our dedicated leads to head our Research and Training agendas.

The CAG provides inpatient and community care for both forensic and developmental psychiatry and has a long and positive history in learning disabilities mental health community provision, and the Estia Centre (our training, consultancy and research arm) continues to deliver an active programme to community stakeholders, careers and health and social care professionals. The CAG has a strong and effective joint partnership with the Institute of Psychiatry psychology and Neurology, with an energetic research and training programme. The Trust is committed to developing members of staff and providing opportunities for local people to make a contribution to mental health care.
