An exciting opportunity has risen for a Band 6 Senior Mental Health Practitioner to join the Community Rehabilitation and Recovery team that supports individuals from across Teesside. The post available is fixed term for 9 months. Due to some recent service improvements you will be joining us at an exciting time to develop the service to meet the requirements of the Community Mental Health Framework (CMHF) and Rehabilitation Services across Tees Valley.

We are looking for an enthusiastic and dynamic individual who are able to work with the wider MDT, voluntary sector and third sector organisations.

Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.

The Community Rehab team operates between 9am and 8pm 7 days a week, 365 days a year.

Main duties of the job

Key skills and responsibilities will include working in rotation with other senior Practitioners within the team to carry out Rehabilitation assessments to determine the needs of the individual and how these can be met by the team. You will be responsible for having oversight of Rehabilitation bed availability and supporting patient flow from inpatient settings as well as working alongside community mental health teams in assessing service user needs for community rehabilitation interventions. This will guarantee continuity of care for service users transiting into and out of rehabilitation services. You will be responsible for providing coordinating and delivery an individualised support package that will enhance their skills and keep them sage and well in the community - this may be as a 'co-worker' or a 'key worker'. Effectively liaising with referring agents will be crucial within your role. The role will involve producing robust treatment reports and recommendations in accordance with Trust and local policies.

It is essential that you exemplify excellent interpersonal skills as they will be paramount when establishing close working relationships with all appropriate agencies and networks in the field of Mental Health, locally, regionally and nationally. There will be an expectation that you will have a small clinical caseload working alongside the Community Rehab and Recovery team in delivering specialist rehabilitation interventions.

Job responsibilities

This role would be befitting for a candidate registered as a mental health nurse, occupational therapist or social worker that would then familiarise themselves and work in the Teesside Community Rehabilitation and Recovery Team.

Applicants must ensure that prior to applying they have agreement from their current line manager that they can be released on a secondment basis. In the event that permanent funding is secured for this post the successful applicant will be confirmed in post on a permanent basis.

Qualifications

Essential

1. Current professional registration with an approved professional body.
2. Evidence of Continuing Professional Development.
3. For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
4. 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
5. Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
6. Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

Desirable

7. Leadership or management qualification
8. Post-graduate qualification in a relevant area

Experience

Essential

9. Significant experience working with people with mental ill health in a care environment.
10. Working collaboratively with service users and their families/carers.
11. Providing clinical supervision to individuals or groups as professionally appropriate.
12. Mentoring or facilitating students on practice placement
13. Working in a multi-disciplinary team
14. Quality improvement activities

Knowledge

Essential

15. Demonstrable knowledge to post-graduate level of evidence-based practice in caring for patients in the designated field.
16. Understanding of relevant legislation (. Mental Health Act, Mental Capacity Act).
17. Care Programme Approach and its application in practice
18. Detailed understanding of Safeguarding and its application in practice
19. Clinical Risk Assessment and Management and its application in practice.
