Vitalis are recruiting for an experienced Social Worker to work as a Team Manager in a Community Integrated Urgent Care team in Tameside.
We are looking for a dynamic and highly motivated Team Manager to join our Community Integrated Urgent Care team to lead and drive forward our service transformation plans in partnership with our Health colleagues and partners across the system. The Community Team consists of Social Workers, Assessors, Physiotherapists, Occupational Therapists, Assistant Practitioners and Care Assistants. IUCT can provide short term wraparound care support for people in their own homes for up to 72 hours.
