Ensuring organisational and professional boundaries support the implementation of evidence based high-quality care and development of services and the workforce. Individualised, direct and indirect specialist nursing care. Specialist teaching locally, nationally and internationally. Co-ordination of research and audit within the Specialised Clinical Services Division (SCSD). Key Duties The postholder will demonstrably improve the care for patient who are deteriorating or have sepsis across the whole Trust, aiming to reduce unplanned admissions to our critical care units whilst providing expert clinical and leadership for the further development of the service. Key components of this post are the provision of expert clinical care, strong clinical leadership, and developing and implementing a strategy for development of the sick, deteriorating and sepsis including the co-ordination of research, audit and quality improvement activity. The postholder is responsible for ensuring that patients receive care that is evidence-based where possible and in accordance with the National Guidelines. The postholder will be expected to have significant input into the implementation of national and local healthcare policies and into developing innovative ways of delivering the service. This will include developing collaborative relationships with other local units both inside and outside the Trust as appropriate to the needs of the service. The postholder will work directly with patients, clients, and communities for more than 60% of their time. The expectations of this job description will be met through professional leadership, consultancy, education and training, service development, audit, research and scholarly activities. Be responsible for holding the Senior Nurse Bleep providing out-of-hours support when on-call. Supporting the manager on-call within normal working hours during the on-call period. Provide leadership to the Hospital Outreach 24/7 Team, acting as an expert resource for developing the service as well as providing clinical supervision to the team. Work with all Health Care Professionals to ensure patients are identified and given early treatment to improve outcome. Work with all members of the adult SCSD to ensure a coherent team approach to the assessment, treatment and ongoing monitoring of patients within and across the hospital. Develop the life after critical care illness service, providing follow up care to patients who have been critically unwell and to make referrals to other professionals where and when appropriate. Be a role model undertaking the expert practice component of the role alongside students (medical and nursing) or nurses who are inexperienced in nurse-led care. Have a comprehensive understanding of, and skills and knowledge to comply with the NMC standards for nurses, including planned consideration of Advanced Practice roles. Collaborate with other members of the SCS Divisional Management Team and to act as a resource for end-of-life care. General Duties The Consultant Nurse Practitioner post will be characterised by the HEE four principles: Expert Practice Professional Leadership and Consultancy Education, Training and Development Practice and Service Development, Research and Evaluation The balance of these functions may vary from time to time in consultation with the Divisional Director of Nursing for SCSD.
