Woodsley & Holt PCN would like to recruit a Podiatristto work within our Primary Care Network (PCN) multidisciplinary healthcare team.

Our ambition is to improve management of patients sufferingwith various foot conditions and other at-risk groups that have limited accessto our community podiatry service.


Main duties of the job

To work as part of the PCN which comprises of 10 localpractices with a total population of around 100,000 registered patients. This role will work closely with other PCN roles: Pharmacists, Physiotherapists, Dietician and SocialPrescriber in addition to the core practice teams.

You will assess, investigate, manage foot conditions andrefer patients who have been directed to your service by a primary healthcareworker or directly by reception where appropriate. You will be an experiencedpractitioner who is comfortable working independently as well as part of awider multidisciplinary team. You will be supported by the clinical team ateach GP surgery and work with the wider clinical team to deliver holistic careto our patients. You will be able to assess and assist in adapting the servicein response to local population needs.


About us

Woodsley & Holt Pak PCN covers thefollowing areas: Woodhouse, Kirkstall, Burley, Hyde Park, Headingley, Cookridgeand Horsforth areas of Leeds, although it is expected the post holders willdevelop and maintain close links across the city.


Job responsibilities

Workas part of a PCNs multi-disciplinary team to clinically assess, treat, andmanage a caseload of patients of all ages with lower limb conditions and footpathologies, using their expert knowledge of podiatry for specific conditionsand topics

Utiliseand provide guidance to patients on equipment such as surgical instruments,dressings, treatment tables and orthotics

Worktowards developing the service to include the options to prescribe, produce,and fit orthotics and other aids and appliances

Providespecialist treatment and support for high-risk patient groups such as theelderly and those with increased risk of amputation

Worktowards developing the service to include supporting patients through the useof therapeutic and surgical techniques to treat foot and lower leg issues e.g.carrying out nail and soft tissue surgery using local anaesthetic

Deliverfoot health education to patients

Implementall aspects of effective clinical governance for their own practice, includingundertaking regular audit and evaluation, supervision and training

Liaisewith PCN multi-disciplinary team, community and secondary care staff, and namedclinicians to arrange further investigations and onward referrals

Communicateoutcomes and integrate findings into their own and wider service practice andpathway development

Develop,implement and evaluate a seamless podiatry support service across the PCN,working with community and secondary care where appropriate, and aimed atcontinuously improving standards of patient care and wider multi-disciplinaryteam working.

Highlyautonomous and independent practitioner.

Clear,polite telephone manner

Knowledgeof IT systems, including ability to use word processing skills, emails and theinternet to create simple plans and reports

Effectivetime management - planning and organising

Demonstratepersonal accountability, emotional resilience and work well under pressure

ProfessionalResponsibilities:

Tomaintain Registration with the Health and Care Professions Council HCPC, andmaintain professional standards of treatment and conduct

Undertakecontinued professional development to understand the mechanics of the body inorder to preserve, restore, and develop movement for patients;

Provideleadership and support on podiatry clinical service development across the PCN,alongside learning opportunities for the whole multidisciplinary team withinprimary care;

Provideeducation and specialist expertise to PCN staff, raising awareness of goodpractice in good foot health;

Ensuredelivery of best practice in clinical practice, caseload management, education,research, and audit, to achieve corporate PCN and local population objectives;

Undertakea range of administrative tasks such as ensuring stock levels are maintainedand securely stored, and equipment is kept in good working order.

Toencourage multi-disciplinary working between all professional disciplines,developing a liaison culture.

Toregularly attend locality multidisciplinary meetings in order to shareinformation on local needs and initiatives and support the development of thePodiatry service.

Toensure all duties are carried out to the highest standard and in accordancewith current quality initiatives within local areas and to meet departmentalactivity targets.

Activelyparticipate in, and show personal commitment to, structured ContinuingProfessional Development [CPD] and a variety of learning opportunities outsidestructured CPD, in order to develop new skills, and maintain and updateexisting skills.

Toensure high quality delivery of the Podiatry service by fulfilling CPDrequirements and objectives identified through the appraisal process.

Toattend all Mandatory Training sessions as required by the PCN


Person Specification


Experience

* An appreciation of the nature of GPs and general practice
* Experience of core indicative patient presentations including but not limited to:
* Skin and nail pathology
* Circulatory disorders affecting the lower limb
* Neurological disorders affecting the lower limb
* Biomechanics
* Rheumatology conditions affecting the lower limb
* Endocrine and metabolic disorders affecting the lower limb
* Care of the Elderly
* Podopaediatrics
* An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners
* Experience of working with multidisciplinary teams
* Involvement in working with and developing departmental protocols/procedures
* Knowledge and experienced in using System One and EMIS


Qualifications

* BSc or pre-reg MSc in Podiatry under a training programme approved by the College of Podiatry
* Registered member of the Health and Care Professionals Council HCPC
* Able to operate at an advanced level of practice
