About the role:

1. Engaging and maintaining engagement with service users.
2. Supporting young adults transition between young people’s services and integrated adult services.
3. Undertaking assessment of individual need and formulating treatment and risk management plans using evidence based assessment and screening tools linked to substance use .
4. Delivering a range of appropriate interventions such as:
• Identification and Brief Advice
• Extended Brief Interventions & structure Psychosocial interventions
5. Provide Harm reduction advice, information and Interventions
6. With oversight of the Service Healthcare and Medical team support young adults to access medical and clinical interventions
7. Delivering relapse prevention interventions.
8. Working alongside families and carers relevant to the young adult’s care plan.
9. Working alongside relevant agencies involved in the young adult’s care plan.

About you:

10. Have a strong understanding of substance misuse issues and experience of working within a similar field
11. Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour.
12. Be an excellent communicator, both verbal and written with good IT skills
13. Proactive and able to work on own initiative with excellent time management and prioritising skills
14. Have a good working knowledge of mental health interventions, services, and good practice
15. Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
16. Ability to manage change successfully in a way that prioritises the needs of service users
17. Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
18. Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
