Clydesdale locality within South Lanarkshire covers the largest geographical area of the four Locality Areas at 512.10 square miles. It has a population of 61,862, with 22.6% aged 65 and over and 10% aged 75 and over. The area includes rural communities (42% of the population living in rural areas), leading to challenges in transport, service access and deprivation. Health and Social Work teams in Clydesdale already collaborate with national integration plans, establishing strong multi‑agency networks and processes to support a person‑centred approach for vulnerable citizens.
As a member of the Locality Adult and Older People’s, Physical Disability and Sensory Impairment (PDSI) team, you will manage your own case‑load, performing physical functional assessments for service users in line with current legislation and funding priorities. You will also carry out regular duty roles within PDSI, providing functional assessments and advice to support independence, and recommend aids and adaptations while considering legislation such as Self‑directed Support and Adult Support & Protection. Your work will be supervised on an eight‑week cycle by the PDSI Operation Manager, and you will collaborate with the wider team to deliver quality service under the Behaviours Framework.
