The Paramedic will play apivotal role in delivering high-quality, responsive healthcare to patients withinthe Practice. Working in close collaboration with GPs and the widermultidisciplinary team, the post-holder will provide the first point ofclinical triage of all incoming requests from patients through our Klinikdigital triage system.
The successful candidate will work as part of ourmultidisciplinary team to assess, diagnose, and treat patients presenting witha wide range of minor illnesses and urgent care needs.Depending on experience and qualifications, the post-holder may also run their own clinics, managing patients independently within their scope of practice.
Paramedics who hold an Independent Prescribing qualification, or who are working towards this, will be supported to utilise and further develop these skills within the role. Applications are welcomed from paramedics at different stages of their career, including those not yet qualified to prescribe.
The role will play a keyrole in improving access to clinical services, supporting chronic diseasemanagement, and contributing to the overall health and wellbeing of our localpatient population.
Main duties of the job
* Work in partnership with GPs to provide high-quality, responsive triage decisions in a timely fashion as set out in the practices PMS contract.
* Undertake face-to-face, telephone, or digital consultations, including running independent minor illness clinics where appropriate.
* Assess patients, obtain detailed medical histories, and conduct comprehensive consultations as required to reach clinical triage decisions and treatment.
* Analyse, diagnose, and explain medical problems clearly and compassionately to patients and families.
* Prescribe medications where qualified as an Independent Prescriber, or contribute to prescribing decisions under supervision and agreed protocols where not yet qualified.
* Discuss with the duty clinician doctor the needs of patients presenting with complex health needs or conditions requiring specific treatment, hospital admission or referral.
* Recommend, arrange, and interpret appropriate diagnostic tests and investigations in line with clinical governance and protocols.
* Formulate evidence-based treatment and management plans, consulting or referring to supervising GP as appropriate.
* Make direct referrals to primary, secondary and social services
* Educate patients and their families about health conditions, treatment plans, and preventative healthcare to promote self-management and long-term wellbeing.
* Accurately document all aspects of patient care, ensuring legal, clinical, and administrative requirements are met.

Sydenham Green Group Practice is a large GP Practice based in Lewisham, providing care to around 15,000 patients from our Health Centre premises which is co-located with community services.
We are a large and friendly team with 10 GPs, 7 nurses, 2 pharmacists and a full administrative and reception team.

Person Specification
Qualifications
* BSc in Paramedic Science or equivalent qualification (Level 6).
* Full HCPC registration as a Paramedic.
* ALS (Advanced Life Support) certification.
* Level 7 modules in Advanced Practice, including Advanced Assessment.
* Level 7 modules in Advanced Clinical Practice
Experience
* Demonstrated experience in clinical decision-making roles.
* Strong clinical assessment and diagnostic skills.
* Sound understanding of UK healthcare regulations and best practice.
* Proficiency with clinical IT systems (preferably EMIS).
* No outstanding investigations or complaints.
* Experience as a clinical mentor.
* Previous experience in primary care or community-based healthcare.
* Experience in end-of-life care and planning.
* Evidence of CPD that expands scope of practice.
* Experience in clinical audit and quality improvement projects.
* Working towards Independent Prescribing qualification
