The postholder will play an essential role in a GP practice and wider medicines management initiatives across LCW services, contributing to safe, effective, and cost-efficient use of medicines.

Ideal for a GPhC-registered pharmacy technician with experience in primary care, hospital, or community settings, and a passion for improving patient outcomes.

Experience with clinical systems, and familiarity with national frameworks (NICE, GPhC, CQC) is essential.

You will be supported to thrive in a role that values patient-first thinking, collaboration, and innovation.

Main duties of the job

The role involves a wide range of responsibilities, including medicines reconciliation, supporting medication reviews, auditing, controlled drug compliance, and prescribing safety initiatives.

The successful candidate will work alongside a supportive multidisciplinary team, including GPs, pharmacists, nurses, and governance leads, with opportunities to shape medicines optimisation work and patient safety strategies.

A strong emphasis is placed on continuous improvement, education, and training, with opportunities to contribute to teaching sessions and quality improvement projects.

* Delivery at GP Practice
* Assist in conducting medication reviews, focusing on patients with polypharmacy and long-term conditions.
* Support the implementation of Optimised Assurance Framework (OAF) standards, including responding to drug alerts and managing safety protocols.
* Work alongside the multidisciplinary team to ensure safe prescribing and medicines optimisation for patients in Planned Care settings.
* Provide technical support for audits and quality improvement projects initiated by the Lead Clinical Pharmacist.
* Perform high-risk drug monitoring reviews, ensuring compliance with safety protocols and addressing any prescribing concerns under the guidance of the Lead Clinical Pharmacist.
* Contribute to the provision of safe and effective pharmacy services.
* Undertake medicines reconciliation in line with practice protocols to improve efficiency and the effectiveness of prescriptions.
* Respond to patients queries face-to-face and via phone, referring complex problems to the pharmacist.
* Participate in patient reviews and update records via clinics, domiciliary visits, and care home assessments.
* Check the accuracy of medicines on prescriptions to ensure compliance and safety.
* Participate in medication audits, identifying areas for improvement and supporting implementation of changes.
* Provide advice to GPs, staff, and patients to support safe prescribing, quality improvements, and cost-effective medicines use.
* Assist in monitoring prescribing budgets and preparing evidence-based resources to support cost-effectiveness.
* LCW-Wide Medicines Management Support
* Assist with the management of prescribed medication, ensuring secure storage, accurate records, and compliance with Home Office and CQC regulations under the supervision of the CD License holder.
* Provide technical support for the investigation of significant incidents or near misses related to medicines, under the guidance of the governance team.
* Support the procurement, storage, and distribution of medicines, ensuring cost-effective and compliant processes.
* Administrative and Operational Support
* Maintain accurate and up-to-date documentation for medicines management processes, including data collection for audits and reporting.
* Act as a point of contact for medicines-related queries, referring complex cases to the Lead Clinical Pharmacist or governance teams as appropriate.
* Assist with the preparation of reports and presentations for governance meetings and training sessions.
* Support with the notes of the medicines management group.
* Education and Training
* Participate in personal and professional development activities, ensuring up-to-date knowledge of medicines management standards.
