About The Opportunity
In Adult Social Care, we’re creating a place where staff feel supported, empowered, and truly valued. Following an ambitious programme of change, we’re launching new services and are now looking for passionate Social Workers, both experienced and newly qualified, to join our Short‑Term Interventions. We are open to candidates who have not yet qualified but are due to this summer.
About The Short‑Term Interventions Team
This dynamic team focuses on prevention and early intervention, helping people stay independent and delaying the need for long‑term support. You’ll work closely with health partners, voluntary organisations, and community groups to deliver strengths‑based assessments and short‑term support that keeps people safe and thriving. You’ll also play a vital role in avoiding unnecessary admissions and supporting safe, timely hospital discharges.
