Overview
We are establishing a Managed Service Network (MSN) model over the next two years to support people with functional conditions in Grampian. This role is funded by NHS Grampian Charity and is part of a small team working through an MSN approach.
Role Summary
The Clinical Practitioner works directly with adults experiencing functional conditions to deliver patient‑centred, goal‑directed therapy, support self‑management approaches and assist patients in navigating services. The role is offered as two full‑time secondment positions for 24 months (part‑time is possible).
Responsibilities
* Deliver patient‑centred, goal‑directed therapy for adults with functional conditions.
* Provide clinical assessment, collaborative formulation, and practical support to help patients manage symptoms and improve day‑to‑day functioning.
* Offer individual appointments and supported self‑management conversations in line with agreed pathways.
* Support development and implementation of pathways, education and self‑management resources to improve patient and professional experience.
