1 - 2 Month Contract With A Local Authority Role Overview We are seeking an experienced Agency Social Worker to join the East Learning Disability Team. The team provides support and care to adults with learning disabilities across the eastern part of Rhondda Cynon Taf. The successful candidate will join a supportive, experienced team and contribute to safeguarding and promoting the wellbeing of service users through high-quality social work practice. Key Responsibilities Undertake social work assessments, care planning, and reviews in line with the Social Services and Well-Being (Wales) Act 2014. Identify outcomes and develop care and support plans, ensuring ongoing monitoring and review. Conduct Mental Capacity Assessments in accordance with the Mental Capacity Act 2005. Assess risks in a balanced way, promoting independence, choice, and co-production. Recognize and promote individual strengths and abilities. Collaborate effectively with other agencies and professionals, adopting a multi-disciplinary approach where appropriate. Maintain accurate, up-to-date electronic records, including case notes, assessments, care and support plans, and reviews, in accordance with departmental policies and procedures. Travel independently across the service area as required. Requirements Qualified and registered Social Worker with relevant experience in adult learning disabilities. Knowledge of relevant legislation, including the Social Services and Well-Being (Wales) Act 2014 and Mental Capacity Act 2005. Ability to assess risks and make balanced professional judgments. Strong collaborative and multi-agency working skills. Effective written and verbal communication skills, with proficiency in maintaining electronic records. Flexibility to work remotely and attend the office as required. Additional Information Bi-Weekly Payments 37 Working Hours Per Week
