Hours: Full-time (although applications for part-time will be considered and flexible working opportunities are available)

Salary: Band 5 equivalent

We are a large, friendly GP Practice in Oxford, with approximately 26,000 patients, looking to recruit an enthusiastic and motivated Pharmacy Technician to join our Pharmacy team. The role is based across both the Manor Surgery and Kennington Health Centre.

The successful applicant will help develop and manage medicines optimisation services and improve the quality and safety of care offered to our patients. It is expected that as part of this role you will enrol on a Primary Care Pharmacy Educational Pathway if you have not completed this already. This is an 18-month programme designed to support you in meeting a range of competencies in order to deliver safe and effective patient care.

You will play an important role within The Manor Group, complimenting the work of our Clinical Pharmacists and Pharmacy Technician as well as the wider multi-disciplinary team. You will help patients get the best from their medicines, use existing protocols to maximise cost-effective preparations, improve repeat prescribing processes (including use of electronic repeat dispensing and online ordering), minimising clinical risk and reducing avoidable wasted medicines.

The role will include visiting our local care home settings with support from our Clinical Pharmacists, to undertake medicines reconciliation, medication reviews and there will be opportunities to shape the medicines ordering processes to reduce medicines wastage. You will work with the Pharmacists to ensure our hospital discharge letters are reviewed in a timely manner and medicines reconciliation is performed accurately to ensure medicine changes are made when patients are discharged back home into the community. This is a patient facing role in Practice, with phone call and occasional face to face interactions with patients to assess medicines use and communicate changes to medicines.

We are looking for someone who is trained as a Pharmacy Technician, with excellent communication skills and a commitment to working as part of a strong and effective clinical team. Job Summary:

To support the Practice based Clinical Pharmacists to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

Key Responsibilities:

· To undertake medicines reconciliation in accordance with practice protocols and to update patients' medical records / prescribing accordingly.

· To support the achievement of the practice's prescribing Quality and Outcomes Framework targets.

· Working with the Clinical Pharmacists and Prescriptions Clerk to contribute to the review and development of repeat prescribing protocols / Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.

· To deal with medication queries including acute medication requests, patient medication and prescription queries. This may involve removing unwanted items, checking review dates, ensuring necessary blood tests and monitoring has been carried out, prescription synchronization etc

· To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.

· To participate in medication audits.

· To assist in the appropriate monitoring and management of prescribing budgets and to liaise with the medicines management team at the Oxfordshire CCG.

· To prepare evidence-based resources and information to support the implementation of rational cost-effective prescribing.

· To support the Practice Based Clinical Pharmacists to provide medication reviews, patient information leaflets, medicine awareness; and to undertake community visits as appropriate, particularly to our local care homes

· To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

· To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols.

· To advise on the sourcing and safe management of medicines as appropriate.

· To assist in the submission of claims for personally administered drugs as required.

· Be responsible for the organisation, planning and of own workload to meet set deadlines.

Professional:

· Agree objectives and a personal development plan and participate in the appraisal process

· Develop and facilitate good working relationships with community pharmacists and other local providers of healthcare

· Plan and organise own workload, including audit and project work, and training sessions for members of the practice team, patients, and carers

· Participate in staff meetings to discuss and implement changes as well as to keep abreast of developments within the Practices and locality

· Attend local, regional and national meetings of relevance as agreed

· Contribute to the Practices development plans

· Maintain high standards, clinically and administratively, in line with CQC requirements

* Complete mandatory training requirements – to include CPR, anaphylaxis, immunisations, safeguarding, information governance
* Maintain professional appearance and professional conduct with colleagues, patients and relatives

· Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality

· Undertake any other duties commensurate with the post holder's grade as agreed with the post holder's line manager

Health & Safety:

* Demonstrate knowledge and application of principles of Health and Safety in the workplace: to include fire, infection control, COSHH, cold chain procedures as per practice policy

· Assist in promoting and maintaining own and others' health, safety and security as defined in the Practice Health & Safety Policies

· Identify the risks involved in work activities and undertake such activities in a way that manages those risks

· Identify and report significant events, following procedures related to professional standards

· Using appropriate infection control procedures, maintain work areas in a tidy and safe way and free from hazards - clean cupboards and restock as per schedule
