Your Role And Responsibilities
As a Reablement Support Worker Team Leader, you’ll be at the heart of our mission to help people regain independence and confidence in their own homes.
You’ll lead a team of Reablement Support Workers, ensuring high-quality, person-centred care is delivered in line with each customer’s Short Term Reablement Plan (STEP). You’ll work closely with colleagues across health and social care to make sure services are responsive, effective, and empowering.
On a typical day, you might:
Develop and review reablement plans with customers, adjusting them as needed to meet agreed outcomes. Supervise and support your team, providing professional guidance and performance feedback. Ensure compliance with health and safety policies, including risk assessments and manual handling. Collaborate with Integrated Neighbourhood Teams, hospital teams, and voluntary sector partners to deliver joined-up care. Contribute to service development and participate in HR processes such as Occupational Health referrals and performance management.
You will need
We hope you’ll bring:
A good level of education (e.g. NVQ Level 3 in Care or Health) or equivalent experience, with a willingness to work towards further qualifications. Supervisory experience or a relevant qualification such as ILM (desirable). Strong leadership skills to manage a dispersed team effectively. A good understanding of the care needs of different service user groups, including older people and those with physical or sensory disabilities. Confidence in assessing staff performance and making informed decisions. A proactive, enabling approach that supports people to achieve their goals.

Home First provides short-term reablement care and support to people in their own homes—often following a hospital stay, illness, or crisis. Our goal is to help people regain independence and avoid unnecessary hospital admissions.
You’ll be part of a passionate, collaborative team that works closely with Adult Social Care Cluster Teams, Independent Providers, Health colleagues, Borough and District Councils, the Multi-Agency Safeguarding Hub, and voluntary sector partners.
