Willow Team, part of the City and North East Oxfordshire AMHT, offers a fantastic opportunity to join a well-established service delivering care to a diverse and vibrant population across Oxford.
Our service includes three locality-based teams aligned with GP practices. You will join a dynamic multi-disciplinary team, working alongside a range of clinical professionals and closely with third sector partners to provide high-quality, holistic, person‑centred care.

This is a full‑time role (37.5 hours, Monday–Friday, 09:00–17:00).
Due to the nature of the role, the ability to travel independently across Trust sites is essential.

Main duties of the job
The role will involve being the allocated key worker for a caseload of patients with severe and enduring mental illness. In these instances, you will be playing an integral role as part of a multi‑disciplinary team in that persons recovery, which would include the assessment, collaborative care planning and review of the persons needs.
You will also be expected to undertake bio‑psycho‑social assessments for patients referred to the team, and this may include offering brief evidence based interventions for up to eight weeks, ensuring to work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well‑being and support their independence and inclusion in the community.
You will also be expected to complete duty, which includes triaging new referrals (we receive referrals from the Keystone Hubs, GPs, EDPS etc) and identifying if the patient requires an assessment within the AMHT or signposting to other services.
You will be expected to be able to complete risks assessments and appropriately escalated any concerns about risk or safeguarding.

Education and Training
Essential criteria
* Professional Qualification (Degree or equivalent)
* Evidence of Continuing Professional Development (CPD) portfolio
* Professional Registration
Desirable criteria
* Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management)
Occupational Experience
Essential criteria
* To have experience of working within a multidisciplinary team
* To have highly extensive experience which should include community and working with mental health
* To have a good knowledge and interest in community care and have specific clinical expertise in mental health
* Understand and have experience of Risk Management
* Experience of supervising other staff
* Experience of audit
Desirable criteria
* Good knowledge of community services
* Awareness of National and local issues concerning mental health
* Experience of supervising student placements.
