Job Summary
An extremely exciting opportunity has arisen for a Patient Flow Coordinator to support the new and transformational One Coventry Integrated Team (OCIT) service. The trust, in partnership with Coventry City Council and the Coventry and Warwickshire Partnership Trust, has designed and developed three Local Integrated Teams (LITs) across Coventry, supported by multi‑professional teams offering a holistic and integrated approach to urgent community needs.
Main Responsibilities
 * Deliver training in pathways and system support tools, ensuring safe discharge planning and accurate patient flow coordination.
 * Support patient flow for those patients case‑managed by Local Integrated Teams.
 * Coordinate assessments and referrals related to patient flow.
 * Communicate with patients, families, carers and care homes to support the LIT processes, including planning meetings as required.
 * Work independently within an LIT, seeking senior-level support for tasks beyond competency.
 * Provide education, support and advice to the LIT on patient flow, discharge processes and pathways.
 * Represent Complex Flow Improvement, lead where applicable, provide team development and caseload updates to managers, and inform the LIT manager of blockages.
 * Escalate and report any concerns via the LIT manager to highlight delays in discharge.
Additional Responsibilities
 * Establish and maintain effective communication with patients, carers, multidisciplinary teams and external agencies.
 * Resolve issues for patients and staff to ensure safe discharge and patient flow, escalating issues as needed.
 * Utilise IT systems to secure accurate, timely patient data and maintain databases in line with audit and data protection guidelines.
 * Communicate all relevant information about patients within the LIT caseload to the multidisciplinary team and ensure accurate recording for handover.
 * Support staff and patients exposed to difficult situations, escalating to the appropriate clinical lead.
 * Promote and monitor adherence to Health and Safety and Trust policies.
 * Work closely with care homes to facilitate flow to their normal place of residence.
 * Maintain a clean, safe environment in accordance with Trust standards of cleanliness, hygiene and infection control.
 * Ensure confidentiality of patient and staff information at all times.
 * Promote awareness of the role of Local Integrated Teams.

Salary and Contract
Band 5 – £31,049 to £37,796 per annum pro rata. Permanent contract, part‑time working pattern.
