Longtown Medical Practice is looking for an experienced nurse to join our team and offer excellent,quality care to our patients. Ideallyyou will be trained in the management of one or more chronic diseases (preferably diabetes) althoughtraining may be available for the right candidate.

Salary is dependant on experience.


Main duties of the job

Our Practice Nurses are expected to provide and maintain a high standard of nursing care for our patients as wellas providing nursing assistance to GPs and other members of the practiceand primary care team.

They will assess,plan, develop, implement and evaluate treatment plans that promote healthand wellbeing, and for patients with long term conditions. They will proactivelyidentify, help in diagnosing and manage treatment plans for patients atrisk of developing a long term condition.

Other duties will include:

* Medication reviews
* Supporting patients to adopt health promotion strategies that promote patients tolive healthy, and apply principles of self-care
* Working with patients presenting with mental health needs inaccordance with the NSF for Mental Health - to be aware of local mental health care pathways, andreferral/support systems.
* Implementing and participating in vaccination and immunisation programmes for both adultsand children
* Meeting the needs of patients presenting with opportunistic wound care.

Where the post holder has completed an approved prescribing qualification they may be asked to prescribe safe, effective and appropriate medication as defined by current legislative framework and within personal competence and agreed formulary boundaries.

Longtown Medical Centre is a small, friendly practice based in Longtown, North Cumbria. We have a practice population of around 3,800 and an experienced and committed team of GPs, Nurse Practitioner, Practice Nurses, HCA and Frailty Co-ordinator, supported by our small but highly effective reception and administration teams.

Job responsibilities

RESPONSIBLE TO: GP Partners (Clinically),Practice Manager (Administratively)

We strive to promote the best evidence based care for our client population in the management of long term conditions and the proactive provision of preventable health care.

Key clinical responsibilities:

* To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs, and other members of the practice and primary care team.
* Assess, plan, develop, implement and evaluate treatment plans that promote health and wellbeing, and for patients with long term conditions.
* Proactively identify, help in diagnosing and manage treatment plans for patients at risk of developing a long term condition.
* Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols.
* Work with patients in order to support adherence to prescribing treatments.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Support patients to adopt health promotion strategies that promote patients to live healthy, and apply principles of self-care.
* Support and manage health needs of women presenting for family planning and cervical cytology consultations.
* Recognise, assess and refer patients presenting with mental health needs in accordance with the NSF for Mental Health. To be aware of local mental health care pathways, and referral/support systems.
* Implement and participate in vaccination and immunisation programmes for both adults and children.
* Advice, support and administer vaccinations where appropriate for patients travelling abroad.
* Meet the needs of patients presenting with opportunistic wound care.
* Where the post holder has completed an approved prescribing qualification for the role and the practice determines that this is a requirement: to prescribe safe, effective and appropriate medication as defined by current legislative framework and within personal competence and agreed formulary boundaries.


Person Specification


Qualifications

* Registered (first level) Nurse.
* Current NMC registration.
* Evidence of post registration education.
* Diploma in chronic disease management.
* Educated at diploma, degree level.
* Cervical cytology, family planning, sexual health qualifications / training.


Experience

* Minimum 2- 3 years experience.
* Primary, community care experience.
* Experience of nurse-led management of long-term conditions.
* Experience of developing and implementing protocols and clinical guidelines.
* Portfolio evidence of continuous professional development.
* Ability to work as part of a team as well as an autonomous practitioner.
* Multi-disciplinary and multi-agency working.


Knowledge, skills and attributes

* Knowledge of accountability of own role and other roles in a nurse-led service.
* Knowledge of health promotion strategies.
* Knowledge of clinical governance issues in primary care.
* Ability to change care management skills to support patients.
* Excellent verbal communications skills, able to communicate difficult messages to patients and families.
* Good written communication skills, clear concise record keeping.
* Time management, able to organise workload.
* Knowledge of national policy standards which inform and have implications for Practice, including NICE, NSF, QOF.
* EMIS clinical system.
* Knowledge of local and national health policy.
* Wider health economy awareness.


Other

* Must be flexible in working hours / able to work at desired times including evenings and Saturdays.
* Own transport.
