What You'll Do
* Craft Personalized Care Plans: Collaborate with residents, families, and healthcare professionals to design and implement care plans tailored to individuals
* Ensure Accuracy: Maintain precise care documentation that is compliant and clear to the team
* Lead with Confidence: Organise and guide the team at the start of each shift
* Facilitate Seamless Transitions: Oversee shift handovers for continuity of care
* Monitor Health: Conduct routine observations and manage health needs with attention to safety
* Administer Medications: Safely manage medications following MAR sheets and protocols
* Build Relationships: Develop trusting relationships with residents, families, and professionals
* Promote Independence: Support residents in daily activities with appropriate assistance
* Act on Feedback: Respond to team input to prioritise resident health and well-being
* Champion Respect: Uphold dignity and individuality of residents
* Encourage Engagement: Facilitate social activities and community involvement
* Prioritize Safety: Maintain a safe environment for all
* Support Nutrition: Monitor nutrition and hydration for residents at risk
* Respond to Emergencies: Act swiftly using on-call systems and reflective practices
About You
* You’re a Registered General or Mental Health Nurse (RGN or RMN) with an active NMC Nursing PIN.
* Care home experience is a plus, but passion for care and improving residents’ lives is essential.
* You connect well with residents, families, and colleagues, bringing warmth and a calm, patient demeanour.
* Communication is your strength—clear, empathetic, and respectful with residents, families, and care teams.
