Senior Patient Safety Lead; Patient Safety Specialist
Job Summary
Promote a safety and quality culture across the Maternity, Gynaecology, Neonatal and Paediatric services, taking a lead in supporting local investigations and responses to incidents in Obstetrics and Gynaecology. Responsible for leading the patient safety agenda and ensuring a unified approach to the review of incidents across the service, in line with the Trust Policies & Procedures and best practice guidance.
Main duties of the job
* Ensure effective governance and risk systems are in place across maternity, gynaecology, neonates and paediatrics. Work closely with the Women and Children’s services management team to ensure effective communication systems and compliance with national and local standards.
* Lead the Patient Safety Team within the PSIRF and collaborate with the Trust Patient Safety Team.
* Act as single point of contact for Maternity & Neonatal Safety Investigations (MNSI). Coordinate documentation upload and investigation interviews.
* Participate as a member of the Perinatal Mortality Reporting Tool Group and attend related meetings.
* Liaise with Professional Midwifery Advocates (PMA) to support staff involved in clinical incidents and investigations.

Responsibilities
* Work with the Women’s services management team and clinical governance leads to ensure all governance arrangements are effective.
* Responsible for all aspects of the clinical governance and risk portfolio, including investigations, audit and complaints.
* Assist in investigation of complaints as appropriate.
* Collaborate with medical, midwifery and nursing teams to implement changes and staff development.
* Provide reports to demonstrate compliance with CQC and NHS Improvement frameworks.
* Report severe brain injury incidents to Head of Legal within 48 hours.
* Notify MNSI for cases meeting Each Baby Counts criteria and maternal deaths.
* Oversee progress against LMNS, ICB plans and transition milestones.
* Provide reports for regional committees and patient safety committees.
* Ensure data quality standards.
* Provide expert clinical advice and guidance on Trust policies.
* Work with ward managers to empower risk assessments.
* Manage and review the risk register.
* Analyse data, present findings to senior managers in various formats.
* Represent Trust at STP Local Maternity System governance meetings.
* Ensure compliance of relevant policies and timely dissemination of safety alerts.
* Provide monthly and quarterly reports on risk, incidents, complaints and trends.
Person Specification
* Essential: NMC registered Nurse or Midwife, significant experience in midwifery, leadership experience, organisational change experience.
* Desirable: Registered midwife, Human Factors training, Masters degree.
Experience
* Essential: significant governance experience.
* Desirable: Masters degree.
Training
* Essential: PSIRF, Patient safety training and governance.
* Desirable: Risk management.
