Overview

We are currently looking for a Community Phlebotomist to work as a member of the District nursing team. You will be trained and supervised by registered professionals, undertaking delegated duties, and expected to follow a plan of care based on the specific needs of service users. You will be required to travel across Fenland to provide a phlebotomy service to support patients in their own homes.


Responsibilities

* The primary activity for this role will be supporting service users with their physical health needs including venepuncture, taking blood pressure, weight and height, and other tasks as required. This will require working across several Neighborhood Teams.
* The post holder will provide support and care for service users with mental and physical health needs and to their carers within a multidisciplinary setting. They will undertake assigned tasks as a lone worker but with support from a registered nurse, including assisting in assessment, planning, implementation and evaluation of nursing care.
* The post holder will be trained to ensure that issues of equality, diversity and rights are dealt with in accordance with good practice and legislation.
* To undertake physical health checks including venepuncture, taking BP, and other tasks as required.
* To work across NT's and community clinics if required.
* Support the team in maintaining essential stocks.
* Promote a positive service user experience through proactive listening, being responsive to issues raised and through resolving issues and concerns raised by service users with colleagues and senior members of the team.
* Be responsible for maintaining and conducting oneself in a professional manner towards service users, carers, colleagues and other agencies, at all times.
* Participate in working collaboratively with other Trust services and external agencies to provide seamless care for service users.
* Observe and report to a registered member of the team any changes which may indicate an alteration in the general wellbeing and safety of a service user.
* Demonstrate empathy, dignity, respect and compassion toward service users at all times.
* Understand the importance of relatives/carers contribution to care and conversing with service users and their relatives/carers, establishing their needs and assisting them as necessary.
* Ensure that entries in the patient electronic health record are current, relevant and accurate.
* Promote and support registered staff with the physical health, comfort and wellbeing of service users, including observations and taking vital signs under guidance and supervision.
* Contribute to the effective running of the service through engaging in clinical discussions and attending neighbourhood team hand-overs.


Qualifications & Experience

* Experience of working in the community setting.
* Experience working as a phlebotomist (full training provided).
* Experience of caring for and supporting adults with complex health needs.
* Enhanced communication and interpersonal skills.
* The ability to work flexibly across the neighborhood team, including travelling for work purposes.
* Computer skills, including the use of health electronic patient records (e.g., SystmOne).
* Good time management skills and the ability to manage individual workload.
* Willing to travel.
