The Role
To provide a safe and effective patient focused specialist clinical pharmacy service to a specified patient group(s) supporting medicines governance and quality improvement activities within specialist area(s) to ensure safe, effective and patient-centred systems in place.
Additional Information Specific to the Role – HEPMA (Hospital Electronic Prescribing and Medicines Administration)
The HEPMA team supports clinicians in all three Lanarkshire acute hospitals (University Hospitals Wishaw, Monklands and Hairmyres) and the off-site beds in NHS Lanarkshire.
The post holder is responsible for supporting the completion of roll out to remaining specialist areas including paediatrics and outpatients.
Subsequent maintenance of HEPMA across NHS Lanarkshire hospitals including:
* Work with other members of the HEPMA team to provide training, ongoing system maintenance and analysis of HEPMA data.
* Undertaking research and complex audits to evaluate benefits realisation on an ongoing basis.
* Participation in the HEPMA on call rota.
To support HEPMA systems to develop practice
* To support the evaluation of the contribution of electronic prescribing to patient care.
* To support the development, delivery and management of the implementation and maintenance of the clinical aspects of HEPMA.
* To support the ongoing research, development and evaluation of HEPMA.
Additional Information Specific to the Role – Frailty
To contribute to the care of patients admitted to the Frailty Unit including provision of pharmaceutical care, medication advice and prescribing.
This exciting new role will contribute to the seven-day Frailty Service within UHW to ensure patients receive high quality pharmaceutical care. The post holder will become an integrated member of the consultant-led multidisciplinary team, liaising with primary care, hospital and community colleagues. There is a requirement for the post holder to be an established practicing independent prescriber due to the complexities of patients presenting with frailty as both prescribing and de-prescribing will be an integral part of this role.
The post holder will contribute to developing the clinical governance agenda and associated systems for medicines use within the service. This will include but limited to:
* Provide pharmaceutical advice for staff working within frailty
* As part of a rota provide clinical pharmacy cover over weekends and public holidays.
* Undertake quality improvement initiatives and where appropriate liaising with key stakeholders
* Facilitate and support processes for medicines communication at the interfaces of care e.g. On discharge from a hospital ward or discharge to the care of the virtual ward or General Practice via an immediate discharge letter
* Liaise with relevant health and social care services as it applies to medicines e.g. social work, acute hospital departments, General Practice, NHSL acute pharmacy services (including off site beds), Community Pharmacy and other key stakeholders.
* Delivery of relevant pharmacotherapy activities e.g. medicines reconciliation, education and quality improvement.
* Participation in multidisciplinary consultant led ward rounds
There will be a focus on optimal use of medicines to ensure a patient focused, efficient progressive, resilient, safe and effective service to frailty.

What we'll need you to bring
* Masters of Pharmacy Degree
* Registered member of General Pharmaceutical Council
* Independent prescribing qualification or must work towards
* Evidence of postgraduate training and professional development in clinical pharmacy practice e.g. Post Registration Foundation Framework/ 1st year clinical diploma
* Relevant post registration experience in clinical pharmacy.
* Ability to effectively monitor patients' therapeutic regimens in order to ensure effectiveness and reduce toxicity
* Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment.
* Commitment to revalidation as per directions of General Pharmaceutical Council
* Good verbal and written communication skills
* Self-motivation
* Participation in weekends and public holiday service
* Participation in pharmacy emergency duty service
* Willingness to contribute to the general working of the pharmacy department.

* Demonstrable experience in providing a clinical pharmacy service to a defined patient group
* Experience in patient information/education
* Experience in supervising staff
* Clinical pharmacy/care planning knowledge and understanding
* Proven project, practice research and audit skills
* Ability to prioritise and organise workload
