To be considered for this position, you must have a full UK driving licence, be willing to drive our minibuses and age 21+. You must also have the flexibility to work in excess of 20hrs per week.

Job Purpose

To create an open and caring environment based on promoting choice, independence, person-led care and experiences. Our endeavour is helping to create community experiences with children, young people, and adults with neurological and physical barriers. This role is part of our Outreach Team, working in partnership with individuals, families, and professionals to create safe, meaningful, and empowering environments. You will help people live purposeful lives, build relationships, and achieve personal goals.

At times, individuals may express feelings of anxiety, frustration, or feeling overwhelmed through behaviour, you will receive full training and ongoing support to work safely and effectively in these situations.

This role is an Outreach Support Worker, which means you will spend the majority of your time out in the community, supporting people in activities, events, and experiences. You may be required to pick individuals up from their home address to attend activities, all are within the Calderdale area. Staff are required to support individuals to engage in a variety of activities, including physical activities such as trampolining and swimming. Staff will facilitate participation, provide guidance, and engage where appropriate, taking into account individual staff circumstances and reasonable adjustments.

Key Responsibilities

· Build trusting relationships with the people you support, respecting their choices, dignity, and individuality.

· Deliver responsive, person-led support in line with individual plans, including PBS strategies.

· Supporting people with activities both in The Hub and in the community, life skills, assisting with communication, mobility and personal care, always promoting independence.

· Encourage participation in community life, hobbies, and personal development activities.

· Keep accurate records of wellbeing, progress, and incidents using agreed systems.

· Work collaboratively with colleagues, families, and external professionals to ensure consistent and holistic support.

· Contribute to the development and review of support plans, risk assessments, and PBS strategies.

· Promote safeguarding, dignity, inclusion, and emotional safety at all times.

· Participate in ongoing training and reflective practice to maintain high standards of care.

Skills & Experience

Must have:

· Full UK driving licence and willingness to drive our minibuses. Age 21 or over for insurance purposes.

· Strong emotional resilience and ability to stay calm in pressured situations.

· Strong communication skills — able to listen actively, speak clearly, and write clear, accurate records.

· Good level of physical fitness to be able to support and actively participate in a wide range of activities.

· A person-led approach, with patience, empathy, and attention to detail.

· Ability to build trusting relationships with the people you support, colleagues, and external partners.

· Organised, motivated, and confident working independently or as part of a team.

· Flexibility to working hours, including evenings, weekends and bank holidays.

Desirable:

· Experience supporting people with learning disabilities.

· Awareness of safeguarding, mental capacity, and health & safety principles.

· Experience working in inclusive, values-led environments.

Qualifications

· Care Certificate or Minimum Level 2 Health & Social Care or Care Certificate (desirable).

· Minimum Grade C English & Math's or equivalent.
