TheSt James Adult Medicine Team are looking to recruit motivated rotationalspecialist clinical pharmacists. We have a host of different rotations foryou to learn from including Older peoples Care, Respiratory and CF, Generalmedicine, Acute medicine, ED, SDEC, infectious diseases and antimicrobialstewardship and Immunology. We have 2 vacancies for pharmacists - 1 is a 18month fixed term contract and the other vacancy is for a permanent contract.

Main duties of the job
Toprovide pharmaceutical care to a group of patients within a clinical areaincluding being an active participant in the MDT, aiding the evolution ofexisting services and supporting training of staff and students.
Toprovide cover for other pharmacists within the relevant Clinical PharmacyTeam as appropriate.
Tosupport the work of the Lead Pharmacist for the area by assisting in theproduction of drug-related policies, procedures and financial reports, and byperforming horizon scanning activities.

Providedrug-related advice and information to patients, consultants and otherhealth-care professionals within a Clinical Management Team.
Monitorthe clinical use of drugs in patients. Advising medical and nursing staff howto optimize drug therapy. Reporting prescribing and administration nearmisses, and risk management issues
Provideeducation to healthcare professionals within the speciality
Participatein the training of junior pharmacy staff within the speciality
Supportthe Lead Pharmacist for the area with the production and implementation ofdrug related policies and procedures, with the production of financialreports and with horizon scanning activities.
Participatein operational service commitments to dispensaries, Medicines Information andaseptic services
Thepost holder will be required to work evenings, weekends and Bank Holidays ona rotational basis and participate in the on-call from home pharmacists rota.
Thepost holder will be required to participate in the core professionalcommitments.

Person Specification
Qualifications
* Pharmacist Registrant of GPhC
* MPharm or equivalent
* Postgraduate Diploma in Clinical Pharmacy (or equivalent)
Experience
* Broad clinical pharmacy knowledge
* Knowledge of hospital pharmacy practice
* Moderate or significant experience of clinical pharmacy
Skills & behaviours
* Team player
* Ability to work under pressure
* Communication skills- written, listening and verbal to both staff and patients.
