The post holder will work as part of the Community Palliative Care Team and undertake a broad range of nursing support duties relating to the care of patients with end of life care needs in the place where they live, usually their own homes.
The post holder will work under the direction of the Palliative Care Team providing care as required to patients with palliative and end of life needs. The post holder will work within the clearly defined boundaries of their competencies.
The key responsibilities of the post include:
* Having a basic understanding of the psychological and spiritual needs of patients and their families and communicating with them as appropriate, but referring to the Clinical Nurse Specialist or District Nurse as necessary
* Establishing and maintaining effective communications with patients, carers, and health professionals in a professional manner
* In line with UHCW documentation policy, updating patient records ensuring entries are accurate, relevant, legible, and timely, and communicating care provided appropriately
* Recognising their own clinical limitations and seeking help appropriately
* Maintaining IT competency in order to make full use of mobile phones, email, the internet, and on-line training
A full UK driving license and access to a vehicle are essential for this post. Please note that an international driving license is not accepted.
