As a Home Care Organiser, you will play a pivotal role in ensuring the seamless delivery of home care services to our clients. You will be responsible for coordinating and organising care plans, liaising with healthcare professionals, and ensuring that our clients receive the highest standard of care tailored to their specific needs.
Key Responsibilities:
1. Develop and implement personalised home care plans in collaboration with clients, their families, and healthcare professionals.
2. Coordinate and schedule home care visits, ensuring timely and efficient service delivery.
3. Liaise with a multidisciplinary team of healthcare professionals to provide comprehensive care solutions.
4. Monitor and assess the ongoing needs of clients, adjusting care plans as necessary.
5. Act as a point of contact for clients and their families, addressing inquiries and concerns with empathy and professionalism.
6. Maintain accurate and up-to-date records of client care and progress.
7. Collaborate with community resources to enhance the support network available to clients.

Additional Information
Full-time position
Salary: £25,000 - £27,000 p.a. (dependent on experience)
