As a Fundamentals of Care Practitioner, you will work closely with specialist teams, ward staff and the wider multi-disciplinary team to ensure the promotion and delivery of clinical care. Focused on enhancing the patient experience through holistic intervention the post holder will work alongside their mentor to develop their own skills and expertise in areas such as:
·wound care
·nail care
·continence management
·falls and deconditioning prevention
·care of people over the age of 65yrs
In addition to this the Fundamentals of Care Practitioner will promote the safety of those patients admitted to the trust through delivery of ward based / focused teaching, participation in clinical audit and contribution prescribed care in the form of risk assessment and care planning.
Main duties of the job
Clinical support role, supporting Clinical Nurse Specialists in the promotion of the Fundamentals of Care, with a focus on Tissue Viability.

Detailed job description and main responsibilities
·To deliver direct care and or provide support and education which is evidence based and individualised to the needs of those patients in their care in areas such as:
Øwound care
Ønail care
Øcontinence management
Øfalls and deconditioning prevention
·To work in partnership with patients to ensure that any intervention provided always promotes privacy, dignity, individual choice and safety
·Where deemed appropriate (via a referral process and in agreement with their manager / mentor) act as a resource to ward staff spending time with patients to:
ØEnsure that all safety promotion measures are in place i.e. care plans
ØPromote gentle activity / exercise
ØEncourage social interaction
ØEncourage and promote independence in those who may require additional time and or staff intervention i.e. those awaiting rehabilitation
·To contribute to the completion of risk assessments, care plans and or other relevant documentation, ensuring counter signatures are obtained where applicable
To participate in team walk abouts, assisting patients on wards with basic care and intervention as required

Training and Qualifications
Essential criteria
1. •GCSE Maths (4 or above or equivalent) •GCSE English (4 or above or equivalent) •GCSE Science (4 or above or equivalent) •A-Levels (equivalent to 112 UCAS points or above / BTEC Extended Diploma (Pass, Merit, Distinction) / Access to Higher Education (112 UCAS points or above)
Desirable criteria
2. •Health and Social Care L2 or above •Study in related fields, e.g biology, child development. Registered B4 Qualification
Experience & Knowledge
