Practice Nurse (Band 5 or Band 6 Equivalent)

Linkwood Medical Practice is seeking a Practice Nurse to join our friendly and supportive team. We are open to appointing at Band 6 equivalent (preferred) or Band 5 equivalent, with clear career progression to Band 6 for the right candidate.
The Role
The post includes a mix of treatment room duties and a strong focus on Chronic Disease Management, delivering safe, evidencebased care as part of the wider general practice team.
Key responsibilities include:
1. Management of longterm conditions (Asthma, Diabetes, CHD, Hypertension)
2. Delivery of preventative health programmes
3. Health screening, patient education, family planning and sexual health services
4. Maintenance of Chronic Disease Registers
5. Supervision of junior nursing staff (Band 6 role)
Band 6 (Preferred)
We are particularly keen to hear from nurses with experience in Chronic Disease Management, ideally including Respiratory care.
Band 5
Band 5 applicants are welcome. We offer structured development and progression to Band 6, supported by training and mentorship.
Essential Requirements
6. Registered Nurse with current NMC registration
7. In current practice, with no restrictions
8. Eligible to work with vulnerable groups (PVG)
Pay and Benefits
9. Practice pay scales aligned closely to NHS Band 5 or Band 6 (depending on experience)
10. 36 days annual leave (including public holidays)
11. NHS Pension Scheme (no break in service)
