The REACH Transplant (Renal Education and Choices @ Home) programme is a Scottish Government initiative to improve equity of access to living donor kidney transplantation (LDKT) in Scotland.
This initiative uses an evidence-based approach of individualised, home education sessions to provide tailored information about living donor kidney transplantation (LDKT) to suitable chronic kidney disease (CKD) patients and their support networks. This personalised approach is intended to overcome known barriers to accessing living donor kidney transplantation as a first treatment option for those who would benefit from transplantation.
The Programme Lead will act as the professional support providing specialist education, induction/training, and leadership to the 5 WTE REACH Transplant Nurse Specialists embedded in renal units in Boards throughout Scotland. Included in the Lead role will be implementing a Quality Improvement initiative, working with clinical colleagues to improve timely transplantation in Scotland.
The Lead will report regular programme audit outcomes to Living Donation Scotland Board and Scottish Government. There is an expectation that this programme attracts national and international attention and the post holder will be expected to present at national meetings and share outcomes with peers and stakeholders.
