Role Overview
You will support service users transitioning from hospital back to their homes, helping them rehabilitate, maximise independence, and live confidently. Duties include personal care, medication administration, monitoring vital signs, rehabilitation support, and liaising with healthcare professionals.
Key Responsibilities
* Deliver care in line with personalised care plans
* Assist with personal care, medication, and rehabilitation exercises
* Monitor vital signs and support domestic tasks
* Liaise with district nurses, pharmacies, and healthcare professionals
* Help clients regain independence and confidence
Candidate Requirements
* Full UK driving licence and access to own vehicle (required)
* Experience in domiciliary/home care preferred (minimum 1 year) but not essential
* Ability to read and follow care plans
* Strong communication skills (verbal and written)
* Basic IT skills for documentation and reporting
* Flexibility to work varied shifts (7:00am–8:30pm)
* GCSE or equivalent preferred
* Paid training and DBS check
* Career progression (Level 2 & 3 Diplomas, specialist training)
