Are you passionate about making a real difference in the lives of vulnerable individuals? We're looking for an Intervention Officer to join our Delayed Transfer of Care – Mental Health (DTOC – MH) Floating Support Team. In this vital role, you'll work directly with customers who are transitioning from hospital or supported accommodation into permanent housing, helping to safeguard their wellbeing, prevent homelessness, and promote independent living.

Key Responsibilities
* Deliver tailored, person-centred support to individuals with mental health needs, enabling them to successfully transition into and maintain secure, long-term tenancies.
* Take an active role in multi-agency forums and case conferences to ensure coordinated support and improved outcomes for customers.
* Identify risks of tenancy breakdown or homelessness early and implement effective, proactive solutions to prevent them.
* Maintain accurate and timely records in line with safeguarding and data protection policies.
* Collaborate closely with internal teams and external partners to deliver consistent, holistic support aligned with individual support plans.
* Promote inclusivity, safety, and wellbeing in all aspects of your work, taking ownership of health and safety issues and embedding equality, diversity, and inclusion into every action.
What you'll need to succeed
* Proven experience supporting people with complex mental health needs, ideally within housing or community-based settings.
* Strong understanding of safeguarding principles and risk management, with a track record of identifying and responding to concerns appropriately.
* Excellent interpersonal and communication skills, with the ability to listen, empathise, and build trust with vulnerable individuals.
* A solution-focused approach and the ability to work independently under pressure, managing multiple priorities and deadlines.
* Confidence using digital tools, mobile devices, and Microsoft applications to record and manage customer data.
* A full valid UK driver's licence, access to your own vehicle, and business use insurance.
