About the Role
We have an exciting opportunity for a pharmacist to join our team and be part of the wider clinical team. We would preferably be looking for full-timer, however, part-time can also be considered.
With extensive experience of working within a primary care setting, the post holder will provide expert support to our GP practices, as well as high quality patient care consultations. The pharmacist will efficiently work as an expert advisor for medicines, repeat prescriptions, medicines optimisation and pharmaceutical care aspects of the multidisciplinary team workload.
Location: Though we prefer this job to be office based at one of our surgeries in Milton Keynes, we would welcome discussions about hybrid or remote working. In case of hybrid working, this will be a non-contractual arrangement; the base office will be the contractual place of work. The number of days that you will be able to work at home will be determined primarily by business needs, but personal circumstances and other relevant circumstances will also be taken into account. If you are successful, any opportunities for hybrid or remote working will be discussed with you prior to you taking up your post.

The job holder will have to:
1. Provide services to patients that are registered with the hub
2. Provide information and support to patients on the following:
3. Medicines Information – General advice requests
4. Pharmaceutical advice
5. Repeat Prescriptions
6. Provide support to our primary care sites and assist with prescribing and high-risk drug management audits as service requirements
7. Use critical thinking and effective communication skills to assess accurately the caller’s needs (establish the level of information required, their existing level of knowledge and other issues including social, cultural and economic factors)
8. Prescribe appropriately for patients requiring urgent prescriptions using access to summary care record information to ensure accurate re-prescribing
9. Communicate information and pharmaceutical advice to callers in a clear and understandable manner, adapted to the needs of the individual
10. Identify and appropriately action clinical risks for health and medicines queries in line with professional responsibility
11. Identify child and adult protection issues arising from health and medicines information calls, and action these in line with protocols
12. Record in a timely manner a true and accurate record of the information and advice given, and to complete an electronic record of the call event using the appropriate clinical system
13. Actively access specialist information by using electronic databases, BNF etc. ensuring accurate advice is given at all times
14. Actively communicate with colleagues to assist service delivery
Therefore, the successful candidate must:
15. Be registered with the General Pharmaceutical Council (GPhC)
16. Have successfully completed CPPE General practice pharmacist pathway
17. Be a Non-Medical Prescriber
18. Maintain professional and clinical competence through mechanisms of continuing professional development
19. Have extensive experience of working within a primary care setting
20. Be able to provide high quality patient care consultations
21. Be able to effectively manage workload and support our services with pharmacy related tasks
