We are seeking an organised, proactive, and experienced Part-Time Care Coordinator to join Feldon Care. The successful candidate will play a key role in coordinating care services, supporting care staff, and ensuring the delivery of high-quality, person-centred care.
Key Responsibilities:
* Coordinating and scheduling care visits efficiently
* Managing staff rotas and ensuring adequate care coverage
* Liaising with clients, families, healthcare professionals, and care staff
* Conducting care reviews, assessments, and care plan updates
* Monitoring service quality and supporting compliance with CQC standards
* Assisting with recruitment, onboarding, and staff support
* Maintaining accurate records and care documentation
* Responding to day-to-day operational and service delivery needs
Requirements:
* Previous experience as a Care Coordinator within domiciliary care or a similar health and social care setting
* Strong organisational and time-management skills
* Excellent communication and interpersonal skills
* Good understanding of care planning, safeguarding, and compliance requirements
* Ability to work independently and manage multiple priorities
* Competent in using Microsoft Office and care management systems
* Up-to-date Mandatory Training certificates
* Right to work in the UK
* Enhanced DBS (or willingness to obtain one)
* Full UK driving licence and access to a vehicle desirable but not essential
