Job overview: The Urgent Community Response Team is a large MDT who aim to respond to physical health crisis in Havering, Barking, Dagenham and Redbridge. This role supports the delivery of high-quality patient care by providing clinical assessment in the patients own home to avoid hospital admission where not necessary.

* Work in a busy demanding role and cope in a crisis.
* Strong clinical reasoning and communication skills to provide holistic assessment in a timely manner.
* Visit patients who may have fallen and be on the floor.
* Contribute to a team with professional curiosity valued.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement – Outer London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
