About the Role
As our Care Coordinator, you will be at the heart of our domiciliary care service, coordinating care packages, supporting staff, and ensuring our clients receive safe, compassionate, and well-organised care in their own homes.
You'll work closely with clients, families, care staff, and healthcare professionals to ensure care is delivered seamlessly, compliantly, and with dignity at all times.
Key Responsibilities
* Coordinate and manage domiciliary and complex care packages, ensuring continuity and quality of care.
* Assess client needs and develop tailored, person-centred care plans.
* Schedule and support care staff to ensure safe, consistent coverage.
* Maintain accurate and up-to-date documentation in line with CQC standards.
* Carry out risk assessments and ensure safeguarding practices are robust.
* Monitor and review care packages, adapting them as clients' needs evolve.
* Build strong relationships with clients, families, local authorities, and healthcare professionals.
* Respond professionally to enquiries, concerns, and feedback, ensuring timely resolutions.
* Support the ongoing development and growth of the domiciliary care service.
Essential Criteria
* Proven experience as a Care Coordinator or similar role within domiciliary and/or complex care.
* Strong knowledge of CQC regulations and compliance requirements.
* Experience supporting individuals with complex needs, including learning disabilities, physical disabilities, or mental health needs.
* Excellent organisational skills with the ability to manage multiple care packages and schedules.
* Confident communicator with strong interpersonal skills.
* Good IT skills, including experience using care management systems.
* A proactive, solutions-focused approach and a genuine passion for quality care.
* Previous experience specifically within domiciliary care services.
* Relevant qualifications such as NVQ Level 3 or above in Health & Social Care.
* Understanding of local authority funding processes.
* Full UK driving licence and access to a vehicle.
* Background in health or social care settings.
