We are seeking compassionate and dedicated Caregivers to provide exceptional support and assistance to individuals in various care settings, including home care, assisted living, nursing homes with reablement, and hospice environments.
The ideal candidate will play a vital role in enhancing the quality of life for clients by delivering personalised care with professionalism and empathy. Duties
* Assist clients with personal care tasks such as bathing, dressing, grooming, and mobility support
* Administer medication following prescribed care plans and medical instructions
* Prepare nutritious meals tailored to individual dietary requirements and preferences
* Support clients with light housekeeping duties to maintain a safe and comfortable environment
* Monitor and document client health status, behaviour, and progress as part of case management
* Provide companionship and emotional support to reduce feelings of loneliness or isolation
* Assist with mobility and transportation needs, including driving clients to appointments or activities
* Follow established care plans and adapt approaches based on client needs and behaviour management strategies
* Recognise signs of medical emergencies or changes in condition, implementing first aid procedures when necessary
* Collaborate with healthcare professionals, family members, and other team members to ensure comprehensive patient care
Experience
* Prior experience in home care, community care or reablement care environments is essential
* Knowledge of dementia care, Alzheimer's care, hospice care, and senior care best practices
* Experience in medication administration and understanding of medical terminology
* Proven ability to handle heavy lifting safely when required for mobility assistance or equipment handling
* Familiarity with developing or following personalised care plans and case management protocols
* Valid driving licence is essential as well as access/ownership to a road worthy vehicle for use while of shift
