Perform medicines reconciliation for patients discharged from hospital including counselling on medicines use and referral to other clinicians as necessary.

Communicate with a wide range of healthcare professionals within the trusts, practices and other services such as care home staff.

Build strong relationships with community pharmacy colleague.

Support patients with their medication and provide advice regarding benefits, risks and side effects.

Support improvements in patient care and the development of integrated pathways across primary and secondary care.
