BACKGROUND

The service provides safe and supported discharge and settling in services to patients from Leighton and Macclesfield hospitals. A seven-day wrap around service ensures that patients who are isolated are supported to return to their home after a period of hospitalisation. Within the support process patients are linked to wider services they may need. Working within a person-centred model the service seeks to ensure the best possible experience for patients that will also serve to reduce avoidable re-admission.
