Responsibilities Conduct assessments to identify clients' needs and challenges.
Develop and implement individualized care plans and intervention strategies.
Provide counseling and support to individuals and families dealing with emotional and social issues.
Advocate for clients' rights and resources within social service systems.
Collaborate with healthcare providers, educators, and other professionals to coordinate care.
Monitor client progress and adjust care plans as necessary.
Participate in community outreach programs to raise awareness about available services.
Document client information and maintain accurate records of services provided.
Requirements Master's degree in Social Work (MSW) from an accredited program.
State licensure as a social worker (LCSW or equivalent). At least 2 years of relevant experience in a social work setting.
Strong understanding of social justice and advocacy issues.
Excellent communication and interpersonal skills.
Ability to work collaboratively in a multidisciplinary team environment.
Crisis intervention experience and strong problem-solving skills.
